It is your responsibility to make sure that the provider you choose is in network with your insurance plan. We will also ask that you make your child's appointment with these specialists and then notify us so that we can obtain the referral authorization from your insurance plan and get it to your specialist's office prior to your appointment. This process can take up to 24 hours for urgent referrals and up to seven days for routine referrals. Subsequent visits should be scheduled at the following ages: 2 Weeks of Age, 2 Months of Age, 4 Months of Age, 6 Months of Age, 9 Months of Age, 12 Months of Age, 15 Months of Age, 18 Months of Age, 2 Years of Age, 3 Years of Age, 4 Years of Age.
